How many islands are there near Bali, and which are worth the trip?
Nine islands are realistically worth a night or more from Bali. The three Nusa islands sit 25 to 45 minutes by fast boat from Sanur, the three Gilis are one to two hours further east, and Lombok, Komodo and Java need a flight. Most trips should pick two, not five.
The mistake almost everyone makes is treating the list as a checklist. Every island here costs you a transfer day at each end, and a ten-day trip that touches four of them is really four days of boats. Two islands, three nights each, is the shape that works.

Which island near Bali should you pick for your first trip?
Nusa Lembongan, for almost everyone. It is the shortest crossing, it is walkable, and it is the only one of the nine where you can arrive without a plan and be fine. Nusa Penida is more spectacular and much harder work, and its roads are the single most common complaint we hear.
If you have three nights or more, do both: they sit a few minutes apart by boat, and Lembongan makes a far more comfortable base for a day on Penida than Penida makes for itself. Our Nusa Penida and Lembongan route pairs them that way.
If you only want one island and you want the water rather than the viewpoints, skip the Nusas entirely and go to Gili Air. The crossing is longer and the payoff is a reef you can swim to from the beach.
What is the difference between the Nusa islands and the Gili islands?
Distance and character. The Nusas are 25 to 45 minutes from Sanur and belong to Bali, with cliffs, viewpoints and rough roads. The Gilis are one to two hours further, belong to Lombok, and are flat, car-free and built around the water. Choose the Nusas for scenery and the Gilis for swimming.

One practical consequence: the Gilis are worth three nights and the Nusas are worth two or three. A single night on either is mostly transfer.
Which island near Bali is best with children?
Nusa Lembongan and Gili Air. Both are small, flat and shallow at the shore, and both have a short walk from the boat to most rooms. Nusa Penida is the one to avoid with young children: the viewpoints have unfenced drops and the island crossings are long and rough.
Gili Meno is the other honest option, calmer than Air but with almost nowhere to eat, which matters more with children than it does without them. On Lembongan, the mangroves and the shallow lagoon at the north end are the part children actually remember.

Can you visit the islands near Bali as a day trip?
Only the three Nusas, and only Lembongan and Ceningan comfortably. The Sanur fast boats make a day trip physically possible to Penida, but the island is big and the roads are slow, so a day gives you two viewpoints and a lot of driving. The Gilis are too far to return the same day.
If a day trip is the only option you have, take the earliest boat out of Sanur and the last one back, and pick one coast rather than trying to cross the island. The west coast viewpoints fill between ten and two, so the early boat is the whole difference.
